Supported Apartment
(Home) Program

Our supported apartment program refers to the services we provide in an individual's apartment or family home. It allows for the most individualization in community living within the agency’s scope of possibilities. The participant, referral source and significant others are involved in determination of the location, staffing patterns, and programming from the first day. In many cases, individuals are transitioning from one of the other programs within ILS, but starting in an apartment or having staff provide services in your family home is also a possibility.

Unit Information:

ILS operates multiple apartment clusters across various existing complexes and locations within Green Bay.  Limited supports are also available in De Pere, Pulaski, and Oconto County, WI. Support varies based on individual needs.

 

Most residents  receive between two and four hours of programming daily, between one and seven days per week.  Overnight staff may also be available.

 

For individuals requiring support beyond these levels, or facing challenges in renting within the community due to ability or rental history, we provide services through our Transitional Apartment (8-plex) Program.

Staffing Information:

Our direct support staff either meet or exceed the State annual training requirements for licensed Home and Community Based Services. 

 

Preparation for and active engagement in community living are emphasized in all aspects of programming. These activities encourage positive choices, exercising good judgment, and establishing support services which are crucial for a successful transition to the community. 

 

ILS works closely with an individual's Support Team and plays a vital role in program development and preparing for any furture transitions to less restrictive environments. We strive to help all participants achieve their goals.
