Adult Family Homes








Programs that are presently available consist of three different state licensed Adult Family Homes - each suited to serve up to four individuals.  Individuals may reside in an Adult Family Home for as many months or as many years as they would benefit from such an environment.  Our Adult Family Homes have shown great success with people transitioning into one of our Supported Apartment programs and eventually living a more independent lifestyle.  All programs provide assistance with individualized programming created to promote independence and continued growth.  A person could begin services with ILS in either of the programs based on their present needs.  

In all homes, transportation may be provided by ILS and is utilized as appropriate for each individual.  During the initial 30 days, 100% supervision may be required so that staff can become familiar with the individual and the individual with us.  Following the 30 days, outings and events are assessed and staff supervision is provided as determined by the program and team members.  As independence is a goal for participants, reduction in staffing patterns is a possibility to see how situations are handled.  Each resident occupies his/her own bedroom and shares common areas (kitchen, living areas, family rooms, and shared bathrooms).  All participants take part in meal preparation and and planning and maintaining a clean environment (vacuuming, dusting, cleaning bathrooms, doing dishes).  If interested, residents are allowed to take part in outside activities as well (flower beds, garden, shoveling, and mowing the lawn).  

The three Adult Family Homes are described in more detail below;

Fisk Street Adult Family Home

The Fisk Street Home is a licensed, 4 bed Adult Family Home designed to serve 4 ambulatory adult women with disabilities that may include Developmental Disabilities, Traumatic Brain Injury, or Chronic Mental Illness.  The house is a 5 bedroom, 3 bathroom, 2-story home that includes a large, private fenced-in backyard.  The home is located in the west side of Green Bay, Wisconsin near the downtown and entertainment areas and within walking distance to a park and public transportation.  Transportation may be provided by ILS and is utilized as appropriate for each individual.  This program does provide 24-hour awake staff at the site, with 1 person on premises at night.  Additional staff are available from a short distance away should an emergency arise.    

Deuster Road Adult Family Home

The Deuster Road Home is a licensed, 4 bed Adult Family Home designed to serve 4 ambulatory adult men 
with disabilities that may include Developmental Disabilities, Traumatic Brain Injury, or Chronic Mental Illness.  The house is a 5 bedroom (one used for site office), 3 bathroom, walk-out ranch style home nestled on a 3 acre parcel adjacent to the Fox River Trail.  It was purchased as a new construction home back in early 2005.  This program operates in a country setting offering fewer neighbors than city living and can more appropriately allow for services to individuals who may demonstrate challenging behaviors.  The home is located in Greenleaf, which is approximately 8 miles south of DePere in Brown county.  24-hour awake staff is provided to the individuals residing in this home, with 1 staff on premises every night.         

Morning Glory Lane Adult Family Home

The Morning Glory Home is a licensed, 4 bed Adult Family Home designed to serve 4 ambulatory adult men with disabilities that may include Developmental Disabilities, Traumatic Brain Injury, or Chronic Mental Illness.  The house is a 5 bedroom, 2 1/2 bathroom, 2 story home located in DePere, Wisconsin.  It is located near the downtown and entertainment areas and is within walking distance to public transportation and a local park that includes a public swimming pool.  Residents can enjoy a large, fenced-in back yard complete with campfire pit.  This program does provide 24-hour awake staff at the site, with 1 person on premises at night.  Additional staff are available from a short distance away should an emergency arise.
